Martin’s Point Health Care and Portsmouth Physical Therapy collaborate to provide free physical examinations to GBCC students seeking to participate in the college-sponsored intercollegiate athletics. The physicals are designed only to determine the student’s fitness to participate in sports and they are not a substitute for ongoing health care by a family physician or other specialist.
A certified athletic trainer is available for all home varsity sporting events at Great Bay Community College.
Requirements for student athletes to participate in varsity athletics:
1. Proof of health insurance.
2. A completed medical history form.
3. A completed medical clearance form.
4. A sports physical within six months of the starting date for your sport. See below for details.
All student-athletes are required to receive a physical to participate in athletics. It is strongly recommended that you receive your physical from team physicians during the designated dates. Documentation of health insurance must be provided at the time of the physical. If you choose to receive a physical from your personal physician, you must have that physician complete and sign all three forms below and submit in to the GBCC athletic department.
All three forms must be read, completed, and turned in to the Great Bay Community College athletic department prior to beginning any varsity sport, including pre-season practices. Any student athlete who does not have the required paperwork completed will be held out of participation until it is received by the GBCC athletic department.
